Default Too positive. Way too positive

What would cause enough PCV to blow the tube off of the head and blow oil all over the top of the head. I made sure it was on there good and tight but after 20 miles I checked it and sure enough it blew off again. New PCV kit installed during a full head gasket and new head replacement. Any help is appreciated.

Clogged oil separator box or incorrectly installed piping to/from the box to the flame trap.

Possible blocked PCV oil line going into the pan
If that opening wasn't thoroughly cleared and cleaned out, all the new parts in the world won't do you any good.
