The Rail Pressure Sensor would have been my next check.

    These are all I did.
    - I cleaned the MAF sensor with a MAF spray cleaner.
    - Switched from the XC70 to this V70XC, still it hesitates.
    - Bought a fuel filter from AutoZone. I did not install it since I could not find what or why the back nipple is slanted, so I returned it.
    - Bought a fuel filter from another place that deals more with European parts. Installed it, took a test drive with high revs, VOILA! It was a smooth drive.

Thank you all for all your inputs. Well appreciated.

Ok, ruled out MAF, see if you can find someone with Volvo diag tool (VIDA DiCE) and run an ETM sweep test to see the condition of your TPS. TPS is a contact based resistive strip that measures the throttle butterfly angle. Over time, the strip gets scratched resulting in wrong angle readings. A dealer / independent can do this in < 1 hour time so figure that for cost.

Another sensor to consider swapping is MAP sensor. It sets on the plastic tube between intercooler and ETM. And since you have a duplicate car, maybe also swap the fuel pressure sensor too?

Cut the shell off the old fuel filter and see how dirty it is.
